Manage Your Network
By integrating patient care in the acute and subacute setting, our Hospitalist becomes the “Captain of the Ship”. Advinity manages your network by directing your patients back to their Medical Home; their PCPs and in-network specialists. If they cannot be seen by their PCP, we will be your patient’s safety net and see them at one of our transitional clinics.
Advinity’s Care Coordinators round daily with our physicians in the hospital to ensure that there are no gaps in discharge planning. We believe that the safest place for our patients to heal is at home with an appointment to see their PCP within 72 hours. If the patient is in need of more in-depth care, such as a Skilled Nursing Facility or Home Health, we will make sure that they have everything they need. Every patient receives a wellness check via phone call within 72 hours of discharge to follow-up on all details the Advinity team coordinated.
We believe that the most important aspect of our work is integrating the patient with their PCP office to prevent miscommunication. Before discharge, your patient will have an Advinity Patient Care Plan explaining diagnoses, procedures, medications, and follow-up appointments. We share Hospital documentation with PCP offices through CareBook, our innovative online platform. Advinity’s philosophy of open communication has helped us earn a 100% satisfaction rating by our patients in an independent hospital survey.
As physicians, it is our duty to document all of our patients’ acute and chronic conditions during their hospital stay. In compliance with CMS mandates to accurately and completely record each patient’s health status, Advinity’s physicians have been educated to paint a complete picture of all active and chronic diagnoses. As a result, we document 4 times the number of conditions compared to the other national Hospitalist Groups. Our efforts have resulted in a 100% improvement on our patient’s risk scores and an average impact on premium of 75% for our current Medicare advantage patients.
Advanced Data Analytics
As an Advinity client you will receive the “Advinity Dashboard”, a dynamic scoresheet that is customizable to your organization’s needs. Our dashboard will give you actionable performance data, such as readmission and observation rates. Of note, our root cause analysis will provide some insight into the real reason your patients are going to the hospital.
Advinity’s biggest impact has proven to be in the area of utilization. Our team has had a readmission rate of less than 10 percent since 2014. In the same period, our client’s overall admissions fell by 20 percent. Advinity ensures your patients are placed in the appropriate status. In our flagship market, our observation rate has risen to over 20 percent. For our current clients, Advinity’s nursing team provides medical reviews for alternate level of care approvals and denials. Without a doubt, we have been a great resource to our partner’s Utilization Management Department.
